By submitting this form, I hereby release and agree to hold the Leadership Experience Tour harmless from and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses, and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the tour, or that may otherwise arise in any way in connection with any services received from the Leadership Experience Tour. I understand that this release discharges the Leadership Experience Tour from any liability or claim that I, my heirs, or any personal representatives may have against the event with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from the Leadership Experience Tour. This liability waiver and release extends to the event together with all owners, partners, and employees.
COVID Release Form
I acknowledge:
The contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing.
I further acknowledge that the Leadership Experience Tour has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.
I further acknowledge that the Leadership Experience Tour cannot guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, event staff, and attendees and their families.
I voluntarily seek services provided by the Leadership Experience Tour and acknowledge that I am increasing my risk of exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending my appointment.
I attest that:
I am not experiencing any symptoms of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
I have not traveled internationally within the last 14 days.
I have not traveled to a highly impacted area within the United States of America in the last 14 days.
I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
I have not been diagnosed with Coronavirus/Covid-19 and have not yet been cleared as non-contagious by state or local public health authorities.
I am following all CDC-recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.
Non-Disclosure
I. Identification of the Parties
This non-disclosure is between Shawn Fair of Fair Consulting and The Leadership Experience Tour and the individual submitting this form.
II. Items deemed confidential
Leadership Experience Tour Program structure and pricing provided within the contract agreement and all discussions during your participation in the Leadership Experience Tour and its affiliate programs are completely confidential.
III. Scope of Confidentiality
Information presented during the Leadership Experience Tour cannot be copied or disseminated to other parties at any time without a written and signed consent form
Leadership Experience Tour Corporate Team, Contractors, and Affiliates.
IV. Exclusions from confidential treatment
This NDA can be excluded for any of the following:
1 - Legal reasons which require the recipient to divulge the information
2 - Information was divulged by another party who has not signed an NDA
3 - Recipient without deliberate intent divulged information shared during the training.
V. Terms of Agreement
This agreement is valid for 5 years from the signed date of June 4th, 2022. Even if the term has ended, the disclosing party isn’t giving up any other rights that it may have under copyright, patent, or other intellectual property laws.
VI. Jurisdiction in case of a dispute
In the event of a dispute between the disclosing party and recipient, the dispute will be handled in Eastpointe, MI, United States, Macomb Count.
VII. Injunction
If deemed necessary by the disclosing party, a court order can be filed stopping the recipient from breaching this agreement.
VIII. No rights in the receiving party
As the Disclosing Part who will share information with the recipient, the recipient has no rights to the ideas or to enter into a deal with the disclosing party.
Media Release Form
I authorize the Leadership Experience Tour Team and their contracted photographer/videographer to capture photo and video footage during the June 2022 Leadership Experience Tour from June 2 – to June 4th, 2022.
The details of this agreement are as follows:
Media (Photos, Videos, Interviews)
By signing this agreement, I understand that photos and video footage will be captured during the event and may be used for the sole purposes deemed appropriate by the Leadership Experience Tour Team and their contracted photographer/videographer.
I understand that photos and video footage captured during this event will in no way be used as a means to collect payment or services.
I understand at no point will any of the photos or video footage be used in a malicious manner to promote embarrassment to those present at the event.
By submitting this form, I agree to have video footage and photos posted to the social media pages and websites associated with the Leadership Experience Tour and their contractors as deemed appropriate
